I am not accepting patients at this time.

Before the pandemic, I was required by law to do first appointments with new patients in person at my office.  After first appointment, patients had the option of being seen by telemedicine videoconference or in person at my office.  For the duration of the pandemic, I will do all appointments by telemedicine videoconference. 

 

To prepare for your first appointment, I must confirm that you can meet these requirements necessary for me to provide quality care.       

 

--Verify by reply email that you have reviewed my website and are in agreement with my policies and procedures.

---Test that your internet and computer can successfully connect with the Doxy videoconference program:    https://doxy.me/cherylwaskiewiczaprn     Click on the link, sign in with your first name and you should be able to see yourself on camera.  There are helpful tips and a "pre-call test" button to help you along.

--Be prepared to print and complete an 8 page New Patient Form and email back to me either by scanning or by taking a photo of each page with your smartphone then send to my email.  I must receive this info before we set the appointment.

--Send a copy of your photo ID (driver's license or passport)

 

--Obtain an upper-arm self-monitoring blood pressure cuff and be ready to demonstrate during the appointment so I can document your blood pressure and pulse.  They can be obtained at costco, amazon, and pharmacies, online or in person.  

 

--You will need to have a credit or debit card to pay for the appointment.  Procedure:  prior to the appointment, go to my website, link below, then to Make Appt/ Payment page and follow directions from there.  Fee $442
